Mindful re-opening. What you need to do.
What you need to bring.
We are pleased to announce that we have received
notice that clinics may transition into providing care for patients
(following closure March 16, 2020 recommended by the regulatory College of
Chiropractors of Ontario to suspend regular practices) The well-being
and safety of our patients and community always comes first. We are
thankful for your understanding. Priority is focused on patients
requiring physical therapeutic interventions to reduce pain/manage symptoms
significantly influencing their quality of life/functioning. We will
be transitioning to seeing priority patients the week of June 1, 2020
(starting June 4). Patients will be informed and screened through our
clinic's telehealth measures, email contacts, and online updates, as well as
downloadable/printable forms (see below). See you soon!
Please familiarize yourselves with the symptoms
of a possible COVID-19 infection, and whether or not you fall into a high
risk category with your age, health conditions, environment (workplace,
occupation, etc.) and family/friend exposures.

Appointments
Call in to book. You will be screened upon
booking, and when you arrive for your appointment, so that you can be
given helpful instructions. Signage on the clinic entrance helps
remind persons/patients to stop before entering and call in with
questions, or for instructions. We will make every effort possible to
contact patients with concerns and questions by phone as a telehealth
measure to assist with physical distancing, reduce exposure risks, and
provide guidance. You can be assured that all safety measures,
including proper sanitization between every patient, the use of barriers for
staff, physical distancing of seating, touch free soap dispensers and
sanitizer stations, as well as appropriate PPE worn by your chiropractor,
will be implemented.

health records by completing a revised healthcare history form downloadable
here [Downloadable Forms].
Please complete these forms and bring them with you on your first
appointment, along with the (unsigned) Informed Consent Forms.
These forms are also available at the clinic
entrance within the building if patients are unable to download the forms.
Having them completed prior to your scheduled visit will save time, and
ensure effective physical distancing within the clinic between patients for
your safety and others.
What you need to bring:
1. Updated healthcare forms you have
printed from our clinic website (2 pages), or have previously picked up
outside our clinic entrance and have filled out at home.
2. Informed Consent Forms (2 pages)
that are unsigned.
3. Cellphone, to contact the reception
before you come in for your instructions.
4. Face covering or mask should be worn
(unfortunately the limited PPE for healthcare professionals prohibits us
from dispensing masks to patients).
